The Space Shuttle was protected by several thousand insulating tiles, most of them shaped to fit one specific place on the airframe and nowhere else. Every reusable vehicle since has had to answer the same question: how do you confirm the heat shield is intact before committing to re-entry, when the damage may be somewhere nobody can reach or see?
Thousands of parts, each in only one place
A reusable vehicle's thermal protection is built from a large number — usually several thousand — of rigid insulating tiles and blankets. They are not interchangeable. Because each tile is shaped to fit both its neighbours and the curved outer surface of the airframe, in many cases a tile can only be placed in one particular location on the vehicle. A replacement is not taken off a shelf; it is made for its spot.
Inspection that cannot be trusted to the eye
Looking is not enough. Assessments were attempted using cameras and by direct visual inspection during spacewalks, but these are not very accurate: a tile can be loose or cracked in ways that simply do not show. The failure mode that matters most — a small breach that lets hot gas into the structure during re-entry — is not reliably visible from outside.
So they watched the launch instead
Since the damage usually happened during ascent, the answer was to instrument the ascent. NASA assembled inspection capability that included high-speed video and precision radar from the ground, a WB-57 high-altitude aircraft recording video during launch, and cameras mounted in the external tank umbilical well returning imagery for analysis on the ground. The heat shield could not be examined properly in orbit, so the moment of damage was documented instead.
